For all commercial motor vehicles, an annual vehicle inspection is required by the Department of Transportation (DOT). This Federal annual inspection is Form 49 CFR. Motor carriers can only use commercial motor vehicles if each section identified in appendix G of the Federal Motor Carrier Safety Regulations has passed an inspection. This is in agreement with the terms of that section during the previous 12 months. It’s important to note these annual vehicle inspection requirements apply to all commercial motor vehicles.

Tax W9 Form – Free Explanation and Printable Form
 
 
Tax W9 forms are used to collect the U.S. Taypayer’s ID. As a business owner, you may come in contact with this form in two circumstances.

    The first is if one of your customers requests that you fill out the form and send it to them so that they can abide by tax law requirements to document their suppliers of products and services.  It lets them report their payments made to others.
     
    The second circumstance is when you need a tax W9 form from the outsources and suppliers you cut checks to during the year.

 
The form is never actually sent to the IRS, but it’s saved by the filer as backup documentation for verification purposes. The information from the W9 and the payments to the provider are usually recorded on a Form 1096 or 1099.

For people searching for a free template of a direct deposit authorization form, we’ve posted one in our free business form templates resource center. Feel free to print it out and customize it for your company. Keep in mind that a proper direct deposit authorization form will include:
 

    An option for at least three different locations to place money.
    Each option should include
    Name of the financial institution
    Account Number
    Routing Number
    A check box to indicate whether the account is a Checking, Savings or Investment Account.
    The amount to place in the account (Net, Percentage, fixed dollar figure)
    A note about what documentation you need in addition to the form, typically a voided check or a copy of the savings account Identification Card.
